ABSTRACT

INTRODUCCIÓN. La disección de aorta es una patología poco frecuente, cuando se presenta, lo hace de una manera catastrófica manifestada por dolor torácico o lumbar intenso acompañado de compromiso hemodinámico agudo, un diagnóstico y tratamiento temprano suelen ser cruciales para evitar la muerte del paciente, así mismo, una disección de aorta puede ser secundario a un trauma torácico por desaceleración y se presenta en 1,5% a 2%, lo que induce sobre todo a un desgarro localizado en la región del istmo aórtico. CASO CLÍNICO. Paciente masculino de 35 años, con antecedente de accidente en vehículo motorizado que provocó un traumatismo cráneo encefálico moderado, hematoma epidural parietal derecho, con resolución quirúrgica hace 18 meses, se descartó lesiones a nivel de tórax con radiografía de tórax normal, además se realizó ecografía FAST en ventana perihepática, suprapúbica, periesplénica y subxi-foidea sin encontrar alteración. Acudió al servicio de emergencia por un cuadro de hemiparesia derecha más disartria de 4 horas de evolución, se realiza tomografía de cráneo sin encontrarse lesión, catalogándose como evento cerebrovascular isquémico con recuperación neurológica. Durante su estancia hospitalaria presenta un cuadro de disnea de grandes esfuerzos y tras realizar exámenes complementarios de control se llega a una conclusión de una cardiomegalia, por lo que es referido a consulta de cardiología donde se realiza ecocardiograma, en la cual se observa disección de aorta ascendente más aneurisma por lo que se decide su ingreso y se instaura tratamiento mediante medidas de soporte, con derivación a hospital de especialidades para cirugía cardiotorácica. CONCLUSIONES. La disección de aorta secundario a un traumatis-mo es poco frecuente, sin embargo, el diagnostico siempre debe estar presente en caso de trauma por desaceleración, ya que es vital para la supervivencia del paciente y su manejo oportuno.


INTRODUCTION. Aortic dissection is an infrequent pathology; when it occurs, it does so in a catastrophic manner manifested by intense thoracic or lumbar pain accompanied by acute he-modynamic compromise. Early diagnosis and treatment are usually crucial to avoid the patient's death; likewise, aortic dissection can be secondary to thoracic trauma due to deceleration and occurs in 1.5% to 2%, which mainly induces a localized tear in the region of the aortic isthmus. CLINICAL CASE. 35-year-old male patient, with a history of motor vehicle accident that caused a moderate head injury, right parietal epidural hematoma, with surgical resolution 18 months ago, chest injuries were ruled out with normal chest X-ray, and FAST ultrasound was performed in perihepatic, suprapubic, perisplenic and subxiphoid window without finding any alteration. She went to the emergency department for a picture of right hemiparesis and dysarthria of 4 hours of evolution, a cranial tomography was performed without finding any lesion, being clas-sified as an ischemic cerebrovascular event with neurological recovery. During his hospital stay he presented with dyspnea of great effort and after performing complementary control tests, a cardiomegaly was found, so he is referred to a cardiology clinic where an echocardiogram is per-formed, which showed dissection of the ascending aorta plus aneurysm, admission is decided and treatment is instituted through supportive measures, with referral to a specialty hospital for cardiothoracic surgery. CONCLUSIONS. Aortic dissection secondary to trauma is rare, however, the diagnosis should always be present in case of trauma due to deceleration, as it is vital for the survival of the patient and its timely treatment.

ABSTRACT

La osteomielitis primaria de esternón es muy infrecuente en niños, con menos de 100 casos publicados hasta la actualidad. Su presentación clínica es a menudo inespecífica, lo que causa un retraso en el diagnóstico. Se presentan dos nuevos casos de osteomielitis primaria de esternón. Ambos referían un cuadro de fiebre, malestar general, dolor torácico y rechazo del decúbito, con eritema preesternal en uno de los casos. La velocidad de sedimentación globular y la proteína C-reactiva estaban elevadas en ambos casos. El diagnóstico se confirmó mediante estudios de imagen y en un caso se aisló Staphylococcus aureus sensible a meticilina en el hemocultivo. Ambos se recuperaron sin complicaciones con tratamiento antibiótico. Debe tenerse en cuenta la osteomielitis primaria de esternón en el diagnóstico diferencial del dolor torácico, especialmente si se acompaña de fiebre, signos inflamatorios locales, intolerancia al decúbito o elevación de reactantes de fase aguda.


Primary sternal osteomyelitis is very rare in children, with less than 100 cases published to date. Its clinical presentation is often non-specific, which results in a diagnostic delay. Here we describe 2 new cases of primary sternal osteomyelitis. Both referred fever, malaise, chest pain, and refusal to lie down, with pre-sternal erythema in one of the cases. The erythrocyte sedimentation rate and C-reactive protein values were high in both cases. The diagnosis was confirmed by imaging studies; methicillin-sensitive Staphylococcus aureus was isolated in the blood culture of one of them. Both recovered without complications with antibiotic treatment. Primary sternal osteomyelitis should be considered in the differential diagnosis of chest pain, especially if accompanied by fever, local inflammatory signs, intolerance to lying down, or increased acute phase reactants.

ABSTRACT

Objetivo: Analisar o uso dos contraceptivos hormonais em mulheres com asma e a escolha desses métodos contraceptivos para essa população, com avaliação de eventuais repercussões sobre novos episódios de asma e sibilos. Métodos: Foram selecionados estudos longitudinais, ensaios clínicos, revisões sistemáticas e metanálises. As plataformas consultadas foram PubMed, Embase, Cochrane e SciELO, com a utilização dos descritores: "contracepção", "contracepção hormonal", "sistema intrauterino liberador de levonorgestrel" e "asma". Resultados: Dois grandes estudos demonstraram que o uso de contraceptivos hormonais esteve associado à redução do risco de novos episódios de asma. Uma revisão sistemática concluiu que os resultados para o uso de contraceptivos hormonais para mulheres com asma foram mistos, com aumento ou redução dos seguintes riscos: novo episódio de asma e aumento da frequência das crises e dos sibilos. O uso da contracepção hormonal em pacientes obesas portadoras de asma é controverso. Conclusão: Os resultados para o uso de contraceptivos hormonais em mulheres com asma são inconsistentes, com relatos de aumento ou de redução do risco de novos episódios. O uso do método contraceptivo deve ser discutido individualmente, levando-se em consideração outros fatores de risco associados e o desejo da mulher. A paciente deverá ser orientada se houver piora dos sintomas clínicos de asma na vigência do uso de qualquer método contraceptivo hormonal.


Objective: To analyze the use of hormonal contraceptives in women with asthma and the choice of this contraceptive method for this population, evaluating possible repercussions on new episodes of asthma and wheezing. Methods: Longitudinal studies, clinical trials, systematic reviews and meta-analyses were selected. Platforms consulted: PubMed, Embase, Cochrane, SciELO, using the descriptors: "contraception", "hormonal contraception", "levonorgestrel-releasing intrauterine system" and "asthma". Results: Two large studies demonstrated that the use of hormonal contraceptives was associated with a reduced risk of new episodes of asthma. A systematic review concluded that the results for the use of hormonal contraceptives for women with asthma were mixed, with increased or decrease in the following risks: new asthma episodes, increased frequency and wheezing. The use of hormonal contraception in obese patients with asthma is controversial. Conclusion: The results for the use of hormonal contraceptives in women with asthma are inconsistent, with reports of increased or reduced risk of new episodes. The use of the contraceptive method should be discussed individually, taking into account other associated risk factors and the woman's desire. The patient will be advised if there is a worsening of the clinical symptoms of asthma while using any hormonal contraceptive method.

Se presentan dos casos clínicos de pacientes jóvenes con dolor torácico agudo, en ellos, el enfoque multidisciplinario y la resonancia magnética cardíaca jugaron un papel crucial en el diagnóstico y tratamiento. Presentación del caso 1. Un paciente de 20 años con dolor precordial y palpitaciones que mostró elevación de los niveles de enzimas cardíacas en los exámenes de laboratorio. La angiografía coronaria no reveló estenosis significativas. Sin embargo, se confirmó el diagnóstico de miocarditis a través de la resonancia magnética cardíaca, lo que llevó al inicio del tratamiento con medicamentos para lograr una función cardíaca adecuada y la prevención del progreso de la enfermedad. Su evolución clínica fue favorable. Presentación del caso 2. Un paciente de 19 años que presentó un dolor torácico intenso que se irradiaba al brazo izquierdo y mandíbula. Los exámenes de laboratorio reportaron elevación de los niveles de troponinas, que generaron la sospecha de un síndrome coronario agudo. La resonancia magnética cardíaca confirmó el diagnóstico de un infarto agudo de miocardio, y la angiografía coronaria reveló una estenosis significativa en la arteria descendente anterior y una ectasia subsiguiente. Durante la hospitalización, se brindó un enfoque terapéutico integral con la administración de medicamentos, monitoreo, control del dolor y prevención de complicaciones, y el paciente mostró una evolución clínica favorable


Two clinical cases of young patients with acute chest pain are presented, where the multidisciplinary approach and cardiac magnetic resonance played a crucial role in diagnosis and treatment. Case presentation 1. A 20 year old patient with precordial pain and palpitations showed elevated cardiac enzyme levels on laboratory examination. Coronary angiography revealed no significant stenosis. However, the diagnosis of myocarditis was confirmed by cardiac magnetic resonance imaging, which led to the initiation of drug treatment to achieve adequate cardiac function and prevention of disease progression. His clinical evolution was favorable. Case presentation 2. 19 year old patient presented with severe chest pain radiating to the left arm and jaw. Laboratory tests reported elevated troponin levels, which raised the suspicion of acute coronary syndrome. Cardiac magnetic resonance imaging confirmed the diagnosis of acute myocardial infarction, and coronary angiography revealed significant stenosis in the anterior descending artery and subsequent ectasia. During hospitalization, a comprehensive therapeutic approach with medication administration, monitoring, pain control, and prevention of complications was provided, and the patient showed a favorable clinical evolution

ABSTRACT

Fundamento: As calcificações de artérias coronárias (CAC) mostram-se como fator preditivo de doenças cardiovasculares (DCV). A tomografia computadorizada (TC) de tórax com protocolo de aquisição de baixa dose apresenta acurácia na identificação de CAC e propicia achados incidentais dessas calcificações, que são comumente negligenciados. Este estudo analisará a prevalência de achados incidentais de calcificação em artérias coronárias em indivíduos não cardiopatas submetidos à TC de tórax. Métodos: Estudo transversal consecutivo de caráter analítico e descritivo. Foram incluídos indivíduos de ambos os sexos que realizaram TC de tórax por encaminhamento, acima de 18 anos e não cardiopatas. A coleta de dados foi realizada por meio de prontuários e ficha de anamnese auto aplicada. As variáveis referentes às CAC e à extensão do comprometimento foram obtidas a partir da reavaliação das imagens de TC de tórax disponíveis no sistema da instituição. Os exames foram anonimizados e avaliados por dois médicos radiologistas experientes. Considerou-se como estatisticamente significativo p≤0,05. Resultados: Foram analisados 397 exames. Encontrou-se prevalência de calcificações em 176 (44%) dos casos. A existência dessas calcificações coronárias está relacionada à idade (p<0,001). As calcificações possuem relação com o sexo (p = 0,03) com maior razão de chance de desenvolvimento em homens (odds ratio [OR] = 1,55). O tabagismo (p<0,001), o sedentarismo (p<0,001), a hipertensão arterial sistêmica (p<0,001), o diabetes mellitus (p = 0,04) e as dislipidemias (p<0,001) mostraram associação positiva. Conclusão: A prevalência de achados incidentais de CAC foi de 44%; variam em maior número entre leve e grave; maior razão de chance no sexo masculino e aumento da prevalência com a idade. Portanto, a TC de tórax mostra-se um efetivo método para avaliar as CAC, e juntamente com a história clínica do paciente pode ser utilizada para medir os fatores de risco para doenças cardiovasculares e intervir no desfecho do quadro.(AU)


Introduction: Coronary artery calcifications (CAC) are shown to be a predictive factor of cardiovascular diseases. Computed tomography (CT) of the chest with a low-dose acquisition protocol is accurate in identifying CAC and provides incidental findings of these calcifications, which are commonly overlooked. This study will analyze the prevalence of incidental findings of calcification in coronary arteries in non-cardiac individuals undergoing chest CT. Methods: Consecutive cross-sectional study of an analytical and descriptive nature. Individuals of both genders who underwent chest CT by referral, over 18 years of age and without heart disease were included. Data collection was carried out using medical records and a self-applied anamnesis form. The variables referring to the CAC and the extension of the impairment were obtained from the reassessment of the chest CT images available in the institution's system. The exams were anonymized and evaluated by two experienced radiologists. P≤0.05 was considered statistically significant. Results: 397 exams were analyzed. A prevalence of calcifications was found in 176 (44%) of the cases. The existence of these coronary calcifications is related to age (p<0.001). Calcifications are related to gender (p = 0.03) with a higher odds ratio of development in men (odds ratio [OR] = 1.55). Smoking (p<0.001), sedentary lifestyle (p<0.001), systemic arterial hypertension (p<0.001), Diabetes Mellitus (p = 0.04), and dyslipidemia (p<0.001) showed a positive association. Conclusion: The prevalence of incidental CAC findings was 44%; vary in greater numbers between mild and severe; higher odds ratio in males and increased prevalence with age. Therefore, chest CT proves to be an effective method to assess CAC, and together with the patient's clinical history, it can be used to measure risk factors for CVD and intervene in the outcome of the condition.(AU)

ABSTRACT

Chest pain is one of the most common complaints in the emergency department. Diseases of the heart, aorta, lungs, esophagus, stomach, mediastinum, pleura, and abdominal viscera can all cause chest discomfort (Gulati et al., 2021; Jiao et al., 2021; Lu et al., 2022). Clinicians in the emergency department are expected to immediately recognize life-threatening chest pain (Jiao et al., 2021). Delayed diagnosis further increases the risk of complications and mortality (Liu et al., 2021). In this case, we present an elderly Chinese female who had a history of myocardial infarction two years previously, with chest pain eventually found to be caused by ingestion of a duck bone.

ABSTRACT

Objective: To analyze the clinical characteristics, diagnosis and treatment of anomalous aortic origin of a coronary artery (AAOCA) in children. Methods: There were 17 children diagnosed with AAOCA from January 2013 to January 2022 in Shanghai Children's Medical Center, Shanghai Jiao Tong University School of Medicine.Their clinical manifestations, laboratory and imaging data, treatment and prognosis were retrospectively analyzed. Results: These 17 children included 14 males and 3 females, with the age of (8.7±3.5) years. There were 4 anomalous left coronary artery (ALCA) and 13 anomalous right coronary artery (ARCA). Seven children presented with chest pain or chest pain after exercise, three patients presented with cardiac syncope, one complained chest tightness and weakness, and the other six patients had no specific symptoms. Cardiac syncope and chest tightness occurred in patients with ALCA. Fourteen children had the dangerous anatomical basis of myocardial ischemia caused by coronary artery compression or stenosis on imaging. Seven children had coronary artery repair, of whom two were ALCA and five were ARCA. One patient had received heart transplantation because of heart failure. The incidence of adverse cardiovascular events and poor prognosis in ALCA group was higher than that in ARCA group (4/4 vs. 0/13, P<0.05). They were followed up in the outpatient department regularly for 6 (6, 12) months; except for the one who lost visit, the rest of the patients had a good prognosis. Conclusions: Cardiogenic syncope or cardiac insufficiency usually occurs in ALCA, and adverse cardiovascular events and poor prognosis are more common in ALCA than in ARCA. Early surgical treatment should be considered for children with ALCA and ARCA accompanied by myocardial ischemia.

ABSTRACT

Objective: To summarize the clinical characteristics of bronchial-pulmonary artery fistula and evaluate the effect of interventional closure of bronchial-pulmonary artery fistula. Methods: A retrospective case study was conducted. Fifteen children with hemoptysis who were diagnosed with bronchial-pulmonary artery fistula in Beijing Children's Hospital, Capital Medical University from January 2018 to March 2022 were selected. Their clinical symptoms and chest-enhanced CT findings were recorded. The children who failed to improve after anti-infection and hemostasis treatment were treated with transcatheter embolization through microparticles under digital subtraction angiography (DSA). The efficacy and post-operation recurrence were evaluated. Results: There were 15 children, including 9 males and 6 females, aged 9.8 (3.7, 12.1) years, weighing 35 (16, 55) kg. There was hemoptysis of varying degrees before surgery. Only 2 children had decreased hemoglobin. Chest enhanced CT showed that their bronchial arteries were thickened and tortuous, including 11 cases of single vessel disease and 4 cases of multivessel disease; 11 children had varying degrees of pneumonia and 4 children had atelectasis. Except for one case effectively treated with medical therapy, the remaining 14 cases were all treated with transcatheter interventional closure with embolic microparticles, among whom 12 had their fistula completely blocked with a single operation and the other 2 children underwent multiple operations because of too many fistulas. One child had extensive bronchial-pulmonary artery fistula which failed to be blocked completely even after multiple operations. Among the remaining 13 children, only 2 patients whose fistula was considered to be completely closed had recurrence presenting with hemoptysis at 3 months and 2 years after the operation, and no hemoptysis was found after the second closure. All children were discharged without chest pain, spinal cord paraplegia, or other serious complications. Fourteen children were followed up for 1.4 (0.9,2.9) years, among whom one still has intermittent mild hemoptysis due to incomplete closure and the rest had a satisfactory outcome. Conclusions: Hemoptysis is the first symptom of bronchial-pulmonary artery fistula. For children with failed medical treatment, transcatheter closure with an embolic pellet is effective, safe and feasible, with a low recurrence rate.

OBJECTIVE@#We aimed to assess the feasibility and superiority of machine learning (ML) methods to predict the risk of Major Adverse Cardiovascular Events (MACEs) in chest pain patients with NSTE-ACS.@*METHODS@#Enrolled chest pain patients were from two centers, Beijing Anzhen Emergency Chest Pain Center Beijing Bo'ai Hospital, China Rehabilitation Research Center. Five classifiers were used to develop ML models. Accuracy, Precision, Recall, F-Measure and AUC were used to assess the model performance and prediction effect compared with HEART risk scoring system. Ultimately, ML model constructed by Naïve Bayes was employed to predict the occurrence of MACEs.@*RESULTS@#According to learning metrics, ML models constructed by different classifiers were superior over HEART (History, ECG, Age, Risk factors, & Troponin) scoring system when predicting acute myocardial infarction (AMI) and all-cause death. However, according to ROC curves and AUC, ML model constructed by different classifiers performed better than HEART scoring system only in prediction for AMI. Among the five ML algorithms, Linear support vector machine (SVC), Naïve Bayes and Logistic regression classifiers stood out with all Accuracy, Precision, Recall and F-Measure from 0.8 to 1.0 for predicting any event, AMI, revascularization and all-cause death ( vs. HEART ≤ 0.78), with AUC from 0.88 to 0.98 for predicting any event, AMI and revascularization ( vs. HEART ≤ 0.85). ML model developed by Naïve Bayes predicted that suspected acute coronary syndrome (ACS), abnormal electrocardiogram (ECG), elevated hs-cTn I, sex and smoking were risk factors of MACEs.@*CONCLUSION@#Compared with HEART risk scoring system, the superiority of ML method was demonstrated when employing Linear SVC classifier, Naïve Bayes and Logistic. ML method could be a promising method to predict MACEs in chest pain patients with NSTE-ACS.

ABSTRACT

El término miocarditis hace referencia a una inflamación del miocardio, que puede tener diversas causas (infecciones, tóxicos, enfermedades autoinmunes). Su diagnóstico es desafiante debido al gran espectro de presentaciones clínicas que puede adoptar, muchas veces imitando patologías más prevalentes como el infarto agudo de miocardio. La miocarditis asociada a enfermedades autoinmunes es poco frecuente, y la importancia de reconocerla radica en que el diagnóstico e inicio temprano del tratamiento son cruciales para mejorar su pronóstico. Presentamos aquí un caso clínico de una perimiocarditis lúpica.


Myocarditis refers to an inflammation of the myocardium, which can have various causes (infections, toxic substances, autoimmune diseases). Its diagnosis is challenging due to the wide spectrum of clinical presentations, often mimicking more prevalent pathologies such as acute myocardial infarction. Myocarditis associated with autoimmune diseases is rare, and the importance of recognizing is that early diagnosis and initiation of treatment are crucial to improve its prognosis. We present here a clinical case of lupus perimyocarditis.


O termo miocardite refere-se a uma inflamação do miocárdio, que pode ter várias causas (infecções, substâncias tóxicas, doenças autoimunes). Seu diagnóstico é desafiador devido ao amplo espectro de apresentações clínicas que pode ter, muitas vezes mimetizando patologias mais prevalentes como o infarto agudo do miocárdio. A miocardite associada a doenças autoimunes é rara, e a importância de reconhecê-la reside no fato de que o diagnóstico precoce e o início do tratamento são cruciais para melhorar seu prognóstico. Apresentamos aqui um caso clínico de perimiocardite lúpica.

ABSTRACT

Introducción: el síndrome de Hamman es la aparición de neumomediastino espontáneo asociado a enfisema, patología poco frecuente en pediatría. Sus síntomas principales son dolor torácico agudo, disnea y enfisema subcutáneo. Objetivo: describir una experiencia clínica en la que se sospechó este síndrome analizando el proceso diagnóstico, estudios complementarios y el tratamiento. Caso clínico: varón de 4 años, con antecedentes personales de laringitis a repetición. Presenta laringitis aguda severa, requirió ventilación no invasiva por 12 horas. A las 24 horas del ingreso instaló de forma brusca estridor inspiratorio, dificultad respiratoria, edema de cuello y cara con crepitación a nivel de cuello y cara anterior del tórax que se fue extendiendo a ambos brazos. Se realiza radiografía de tórax que evidencia enfisema subcutáneo extenso. Se realiza tomografía de cuello y tórax que descarta lesión en vía aérea. Con el planteo de síndrome de Hamman en un niño que presentaba como factor predisponente el episodio de laringitis aguda, se decidió no continuar con estudios complementarios. Se realizó tratamiento fisiopatológico con buena evolución clínica. Alta a la semana del ingreso. Conclusiones: el diagnóstico en la urgencia del neumomediastino espontáneo puede representar un desafío, con una variedad de diagnósticos diferenciales potencialmente mortales. La alta sospecha permitirá realizar una evaluación clínica e imagenológica adecuada que permitan confirmar su diagnóstico y evitar procedimientos invasivos innecesarios.


Introduction: Hamman's syndrome involves the appearance of spontaneous pneumomediastinum associated with emphysema, a rare pathology in pediatrics. Its main symptoms include acute chest pain, dyspnea and subcutaneous emphysema. Objective: to describe a clinical case of this suspected syndrome by analyzing the diagnostic process, supplementary studies, and treatment. Clinical case: 4-year-old male with personal history of repeated laryngitis. He presented severe acute laryngitis, requiring non-invasive ventilation for 12 hours. Twenty-four hours after admission he developed abrupt inspiratory stridency, respiratory distress, neck, and face edema with crepitation at neck level and front of thorax spreading to both arms. A chest X-ray shows extensive subcutaneous emphysema. A neck and chest CT scan rules out airway injury. With the suggestion of Hamman's syndrome in a child who presented acute laryngitis as a predisposing factor the episode, it was decided not to continue with additional studies. Physio-pathological treatment was carried out with good clinical evolution. He was discharged a week after admission. Conclusions: diagnosis of spontaneous pneumomediastinum can be challenging in an emergency setting and having a variety of potentially life-threatening differential diagnoses. High suspicion levels will enable an adequate clinical and imaging assessment to confirm its diagnosis and avoid unnecessary invasive procedures.


Introdução: síndrome de Hammam é o aparecimento de pneumomediastino espontâneo associado ao enfisema, uma patologia rara na pediatria. Seus principais sintomas são dor aguda no peito, dispneia e enfisema subcutâneo. Objetivo: descrever um caso clínico com suspeita dessa síndrome por meio da análise do processo diagnóstico, estudos complementares e tratamento. Caso clínico: menino de 4 anos com histórico pessoal de laringite recorrente. Teve laringite aguda grave, o que exigiu ventilação não invasiva por 12 horas. Dentro de 24 horas após a internação, instalou-se abruptamente estridor inspirador, dificuldade respiratória, edema do pescoço e rosto com estalos ao nível do pescoço e face anterior do peito, estendido para ambos braços. Raios-x do tórax mostram enfisema subcutâneo extenso. Tomografia do pescoço e tórax exclui lesões nas vias aéreas. Com a sugestão da síndrome de Hammam em uma criança que apresentou como fator predisponente o episódio de laringite aguda, decidiu-se não continuar com estudos complementares. O tratamento fisiopatológico mostrou boa evolução clínica. A alta teve lugar dentro de uma semana da data da internação. Conclusões: o diagnóstico na urgência do pneumomediastino espontâneo pode representar um desafio, além do mais quando existem uma variedade de diagnósticos diferenciais que ameaçam a vida. A alta suspeita permitirá uma avaliação clínica e de imagem adequada para confirmar o diagnóstico e evitar procedimentos invasivos desnecessários.

Abstract Introduction: Chest binders are an important resource for building masculinity in transgender men without mastectomy, although they can cause respiratory and thoracic damage if misused. Objective: To analyze the association between chest binding and chest complaints in transgender men. Methods: This was a quantitative cross-sectional study conducted at the Trans Space of the Hospital das Clínicas de Pernambuco and the LGBTQI Patrícia Gomes outpatient clinic of the Lessa de Andrade polyclinic in Recife (Pernambuco State, northeastern Brazil). Sixty transgender men aged >18 years were included, and data on sociodemographics, general health, chest binding, and respiratory complaints in the thoracic region were collected. Descriptive analysis was performed, and prevalence ratios (PR) were estimated with a confidence interval (CI) of 95% to associate between binder use and complaints in the chest region. Results: The average was 27.25 years, of which 81.7% did not undergo mastectomy and 53.3% used chest binders. The use of chest binders was significantly associated with complaints in the chest region (PR = 2.73), difficulty breathing (PR = 2.27), and chest pain (PR = 1.82). Conclusion: This study demonstrated a higher prevalence of complaints in the chest and respiratory region in transgender men who use chest binders. This reinforces the need to broaden the view on the health of this population. Gender construction strategies are essential for the quality of life and mental health of transgender men, and binders are an important ally in this process.


Resumo Introdução: O uso de binder é um importante recurso para a construção da masculinidade nos homens transexuais não mastectomizados, contudo, pode acarretar danos respiratórios e torácicos se usado de forma inadequada. Objetivo: Analisar a associação entre o uso de binder e as queixas torácicas em homens transexuais. Métodos: Estudo transversal quantitativo realizado no Espaço Trans do Hospital das Clínicas de Pernambuco e no ambulatório LGBTQI Patrícia Gomes da Policlínica Lessa de Andrade, em Recife, PE. Foram incluídos 60 homens transexuais, com idade acima de 18 anos, e foram coletadas informações sociodemográficas, de saúde geral, uso de binder e queixas na região torácica. Foi realizada a análise descritiva e estimadas as razões de prevalência (RP) com intervalo de confiança (IC) de 95% para a associação entre o uso de binder e queixas na região do tórax. Resultados: A média foi de 27,25 anos; destes, 81,7% não realizaram mastectomia e 53,3% faziam uso de binder. O uso de binder teve associação significativa com queixas na região do tórax (RP = 2,73), dificuldade para respirar (RP = 2,27) e dor no tórax (RP = 1,82). Conclusão: Este estudo demonstrou que há prevalência maior de queixas na região do tórax e queixas respiratórias nos homens transexuais que fazem uso de binder. Isto reforça a necessidade de ampliar o olhar sobre a saúde dessa população. Estratégias de construção de gênero são essenciais para a qualidade de vida e saúde mental de homens transexuais, sendo o uso de binder um importante aliado neste processo.

La COVID 19 es una enfermedad que, habitualmente, no resulta grave en la edad pediátrica, excepto en niños con comorbilidades significativas subyacentes. Es muy importante reconocer los cuadros post COVID, como el síndrome inflamatorio multisistémico (SIM-C) y la COVID-19 prolongada o long COVID que pueden afectar de manera significativa a la población de niños y adolescentes. La pandemia COVID-19 también ha tenido un fuerte impacto en los aspectos sociales, emocionales y nutricionales. El aislamiento prolongado impactó en los controles de salud de niños y adolescentes con enfermedades crónicas y las coberturas de las vacunas de calendario disminuyeron significativamente. Es claro que la vacunación contra el SARSCov-2 en niños, niñas y adolescentes no sólo busca cuidar de su salud; también busca preservar la vida social y presencialidad escolar, reducir el riesgo de los cuadros post COVID-19 y mejorar la inmunidad de rebaño de la población general.

It is clear that vaccination against SARSCov-2 in children and adolescents not only seeks to take care of their health , it also seeks to preserve social life and school presence, reduce the risk of post-COVID-19 conditions and improve herd immunity in the general population.

Se presenta el caso de una paciente de 56 años, quien consulta por dolor torácico lateral izquierdo de dos días de evolución y en su radiografía de tórax tomada en preoperatorio de nódulo tiroideo aparece una pequeña banda de atelectasia plana basal izquierda. Por persistencia del dolor en el posoperatorio tardío, se realizan estudios de ultrasonido, gammagrafía ósea, TAC de tórax y RNM de tórax, encontrándose finalmente un elastofibroma dorsi como causa del dolor a partir de los estudios realizados. El elastofibroma dorsi es una entidad relativamente rara, de patogénesis desconocida, que requiere de sospecha clínica para su diagnóstico. Las imágenes pueden comprobar el diagnóstico sin requerir biopsia. La resección es necesaria por crecimiento de la lesión o dolor persistente. Se revisa la literatura pertinente.


Here we present a case of a 56-year-old female patient, who consulted for presenting left lateral chest pain of 2 days of evolution, and in her chest X-ray taken preoperatively for a thyroid nodule, a small band of flat left basal atelectasis appears. Due to persistent pain in the late postoperative period, thoracic ultrasound, bone scan, chest CT, and chest MRI studies were performed, finally finding an elastofibroma dorsi as the cause of the pain. The studies carried out are shown. Elastofibroma dorsi is a relatively rare entity of unknown pathogenesis that requires clinical suspicion for its diagnosis. Imaging can confirm the diagnosis without requiring a biopsy. Resection is necessary due to growth of the lesion or persistent pain. Relevant literature is reviewed.

RESUMEN Introducción: el dolor torácico agudo es una sensación dolorosa que se manifiesta entre el diafragma y la base del cuello. En Cuba, constituye una de las causas más frecuentes de consulta médica. La provincia de Matanzas muestra un comportamiento similar. Objetivo: caracterizar el perfil clínico de los pacientes con dolor torácico agudo en la Unidad de Cuidados Intensivos Emergentes, del Hospital Universitario Clínico Quirúrgico Comandante Faustino Pérez Hernández, de Matanzas. Materiales y métodos: se realizó un estudio transversal descriptivo con las historias clínicas de 418 pacientes que acudieron a la Unidad de Cuidados Intensivos Emergentes, refiriendo dolor torácico agudo, de enero a diciembre de 2019. Resultados: el rango de edad más afectado fue el de 50 a 59 años, para un 30,38 %. Predominó el sexo masculino con el 30,08 %. El ejercicio físico intenso elevó el riesgo de aparición de dolor torácico. Las primeras causas de dolor torácico agudo fueron, en orden de frecuencia, la osteocondritis esternocostal, la bursitis del hombro y el síndrome coronario agudo. Conclusiones: se evidenció que en la mayoría de los casos el origen del dolor fueron causas no cardiovasculares, afectando más al sexo masculino. Existió asociación estadística significativa entre la actividad física intensa y el inicio del dolor torácico agudo. Egresaron vivos de la unidad el 71,53 % de los pacientes. Se recomienda priorizar la atención de los pacientes que refieren dolor torácico agudo, para una correcta clasificación y atención en el menor tiempo posible (AU).


ABSTRACT Introduction: acute chest pain is a painful sensation perceptible between the diaphragm and the base of the neck. It is one of the most frequent causes of medical consultation in Cuba. It shows a similar behavior in the province of Matanzas. Objective: to characterize the clinical profile of the patients with acute thoracic pain in the Emergency Intensive Care Unit of the Teaching Clinic-Surgical Hospital Comandante Faustino Perez Hernandez, of Matanzas. Materials and methods: a descriptive, cross-sectional study was carried out with the clinical records of 418 patients who attended the Emergency Intensive Care Unit referring acute thoracic pain, from January to December 2019. Results: the most affected age range was the one from 50 to 59 years, for 30.38 %. Male sex predominated, with 30.08 %. Intense physical exercise raised the risk of thoracic pain. The first causes of acute thoracic pain were, in order of frequency, sternocostal osteochondritis, shoulder bursitis, and acute coronary syndrome. Conclusions: It was evidenced that in most of the cases the source of the pain was non-cardiovascular causes, more affecting the male sex. There was significant statistic association between intense physical activity and acute thoracic pain. 71.53 % of the patients was discharged from the unit alive. It is recommended to prioritize the attention of patients referring acute thoracic pain, for their correct classification and care in the shortest possible time (AU).

El Síndrome coronario agudo (SCA) se define como la obstrucción brusca de una arteria que puede dar lugar a una isquemia miocárdica aguda que se acompaña de un síndrome clínico característico que puede ir desde una isquemia con elevación o sin elevación en el segmento ST, angina estable o inestable y muerte súbita. Dado que el SCA es considerado un problema mundial por su alta incidencia y una de las principales causas de muerte es que resulta indispensable la creación y aplicación de un protocolo de recepción del paciente con SCA donde el enfermero que recepciona al paciente con dolor torácico en un servicio de urgencia pueda realizar la valoración de forma oportuna y rápida teniendo en cuenta una secuencia de intervenciones y cuidados que se encuentren plasmados en una planilla el cual garantice la implementación de las medidas terapéuticas a tiempo, aumentando la eficacia de las mismas, reduciendo la morbimortalidad y disminuyendo los costos hospitalarios. El objetivo del protocolo es estandarizar las intervenciones y cuidados de enfermería en la atención inicial del paciente con SCA[AU]

Resumen Objetivo: Evaluar la utilidad del índice UDT-65 para la estratificación del dolor torácico en urgencias en una población colombiana en la que se sospecha enfermedad coronaria. Método: Se condujo la validación externa del índice UDT-65 en una cohorte concurrente que incluyó pacientes que ingresaron a urgencias de una clínica cardiovascular en Bogotá con dolor torácico no traumático, y electrocardiograma normal o no diagnóstico. Se evaluaron 1320 pacientes de 18 o más años y se determinó la utilidad del índice en términos de calibración (uso de gráfico, ji al cuadrado para datos agrupados y prueba de bondad de ajuste de Hosmer-Lemeshow) y de capacidad de discriminación del modelo (curva de características operativas del receptor [ROC] y área bajo ella [AUC]). Resultados: El índice UDT-65 en esta población suministró evidencia de su utilidad en términos de calibración y capacidad de discriminación, para efectuar una buena aplicación de él en aquellos pacientes que consulten al servicio de urgencias de una clínica cardiovascular por dolor torácico no traumático de posible origen coronario. La capacidad de discriminación del índice UDT-65 fue adecuada, pues con un área bajo la curva ROC de 0.867 (IC 95% 0-847-0.885), que se acerca al valor obtenido (AUC 0.87) en la población española en que se desarrolló el índice. Conclusiones: Se necesitan más estudios similares en otras instituciones, dado el buen resultado, en beneficio de más pacientes.


Abstract Objective: To evaluate the clinical usefulness, in the emergency service, of the UDT-65 index for chest pain stratification in colombian population with suspected coronary disease. Method: The external validation of the UDT-65 index was conducted in a concurrent cohort that included patients admitted to the emergency service of the cardiovascular clinic in Bogotá with non-traumatic chest pain and normal or non-diagnostic electrocardiogram. 1320 patients were evaluated and the usefulness of this instrument was determined in terms of calibration (use of graph, Chi-square test for group data and the Hosmer-Lemeshow goodness-of-fit test) and discrimination capacity of the model (curve of receiver operating characteristics [ROC] and by finding the area under the curve [AUC]). Results: The UDT-65 index in the population under study, provided evidence of its usefulness in terms of calibration and discrimination capacity; this, in pursuance of a good application of the instrument in those patients who consult the Emergency Department of the cardiovascular clinic for non-traumatic chest pain of possible coronary origin. The discrimination capacity of the UDT65 index was adequate, with an area under the ROC curve of 0.867 (95 % CI 0.847-0.885), which is close in value to the one obtained (AUC 0.87) in the Spanish population where the original index was developed. Conclusions: More similar studies are needed in other institutions, due to the excellent and beneficial outcomes.

El síndrome coronario agudo (SCA) es la principal causa de muerte, por esta razón, es fundamental reconocer sus características clínicas. Tradicionalmente ha sido descrito un cuadro denominado típico, consistente en dolor torácico retroesternal o en la región izquierda del tórax, explicado como una sensación de presión o pesadez, de duración superior a 20 minutos, que se puede irradiar a la extremidad superior izquierda o derecha, cuello o mandíbula, asociado a diaforesis y náuseas. Diversos grupos de pacientes como mujeres, diabéticos, ancianos y con antecedentes de falla cardiaca o accidente cerebrovascular presentan cuadros llamados atípicos, que en gran proporción no manifiestan dolor torácico. Varios estudios plantean que no hay síntomas suficientemente sensibles y específicos para ser considerados típicos en el contexto del SCA, por lo que el diagnóstico debe incluir además de la evaluación clínica, el electrocardiograma, los factores de riesgo y los biomarcadores. El uso de puntajes de riesgo como el HEART ha demostrado ser útil en este escenario.


Acute coronary syndrome (ACS) is the main cause of mortality around the world; the-refore, it is essential to recognize the clinical characteristics that increase its diagnostic suspicion. Traditionally, it has been defined as a so-called typical condition, consisting of retrosternal chest pain or pain in the left region of the thorax. It is described as a feeling of pressure or heaviness, lasting more than 20 minutes, which can radiate to the left or right upper limb, neck or jaw, and can be associated with diaphoresis and nau-sea. Various groups of patients such as women, diabetics, the elderly, and those with a history of heart failure or cerebrovascular accident, have so-called atypical clinical sets of symptoms, which in a large proportion may not present chest pain. Various studies suggest that there are not sufficiently sensitive and specific symptoms to be considered typical in the context of ACS, thus, the approach must include the electrocardiogram, risk factors and biomarkers in addition to the clinical evaluation. The use of risk scores such as the HEART has proven very useful in this scenario.

Tradicionalmente, é descrita uma condição dita típica, que consiste em dor torácica retroesternal ou na região esquerda do tórax, explicada como uma sensação de pressão ou peso, com duração superior a 20 minutos, que pode irradiar para a esquerda ou direita membro superior, pescoço ou mandíbula, associado a sudorese e náusea. Vários grupos de pacientes como mulheres, diabéticos, idosos e com história de insuficiência cardíaca ou acidente vascular cerebral apresentam condições atípicas, que em grande parte não manifestam dor no peito. Vários estudos sugerem que não existem sintomas suficientemente sensíveis e específicos para serem considerados típicos no contexto da SCA, de modo que o diagnóstico deve incluir, além da avaliação clínica, o eletrocardiograma, fatores de risco e biomarcadores.
